What to bring to your first appointment
Please arrive ten minutes early for your first appointment to leave time to complete our new patient forms. You’ll also need:
- Name of your family doctor
- List of current medications
- List of current health issues
- Your health insurance card and booklet
Insurance
We provide direct billing to your health insurance company. Please note that you are responsible for the full cost of your treatment, including any amounts not covered by insurance.
Payment Options
Payment is due at the time of your treatment. We accept Visa, Mastercard, American Express, and debit.
